Policy-to-Practice Check

Choose one obligation, risk or recurring process and trace it from paper to practice. The purpose is to find gaps early and kindly, not to catch anyone out.

  1. 1.Obligation or risk being examined:

  2. 2.Owner (the person or role accountable for it):

  3. 3.Current policy and its last review date:

  4. 4.Is the policy accessible and understood by the people who need it? How do you know?

  5. 5.Procedure or everyday playbook that translates it into practice:

  6. 6.Training: who is trained, how, and how often?

  7. 7.Evidence: what records show it is actually happening?

  8. 8.Where does everyday practice depart from the policy, and why?

  9. 9.Learning and review: what would trigger a change, and who would make it?

This worksheet is independent educational material from Spirit Led Institute. It is a reflective tool for learning, not a compliance audit, legal opinion, WHS assessment, safeguarding investigation or psychological assessment. Laws and obligations vary by jurisdiction and change over time; check current authoritative requirements and obtain appropriately qualified advice where necessary.
